Secure order form for Personalised Woven Name Tags

Therse labels are 10mm high and the length is determined by the number of letters and type style chosen.
They are supplied in strips of about ten labels.Top and bottom of label is sealed and you cut from the strip.
Select Typestyle and enter appropriate letter in form below: The type styles as shown can not be altered in any
way, including the placement of capital letters. The wording will be produced as per type style chosen.
Only on the double line HA can the letters be reversed to AH. To give Upper & lower case on the top line
and all upper case on the bottom line.

Australian delivery : for sew on labels is approximately 2 to 3 weeks from receipt of order and 4 weeks for iron on .Postal cost $3.00
Overseas delivery : for sew on labels is approximately 3 to 4 weeks from receipt of order and 5 weeks for iron on. Postal cost US $5.50
These are custom made items. Payment details must be submitted with order and labels will be charged for prior to production of order.
This is a firm order and once entered into our production system it can not be cancelled. Orders are processed on receipt.
